

Policies and Informed Consent
Confidentiality
All services and records are kept confidential in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Every effort will be made to protect your personal information and information will not be shared without your written permission. You have the right to revoke any such permission verbally or in writing at any time.
**Limitations and Exceptions of Confidentiality **
This therapist is required by legal statute to break confidentiality in the following circumstances:

The receipt of a subpoena or court order asking for the release of your records

Please note that efforts will be made to quash these requests, but this therapist may still be asked to comply with the subpoena

There is a disclosure of abuse/neglect or suspected abuse/neglect involving the endangerment of a child or an incapacitated adult

There is a direct threat of violence and intention to act against self or a third party. This therapist has a duty to warn that third party in this instance. In the event of active suicidal ideation this writer may break confidentiality in order to keep the client safe.
Clinical Consultation
This therapist seeks clinical consultation from colleagues in the community in order to ensure best care for clients as needed. This includes providing information pertinent to specific questions this writer may want outside guidance and perspective on. No identify information will be shared in order to ensure client confidentiality is maintained. At any time the client can request for this writer not to discuss any aspects of their treatment during clinical consultations.
Communication
Primary form of communication should be limited to messaging via Simple Practice Client portal. This is to ensure highest level of HIPAA compliance with secure encryption in order to protect your personal information. This can be accessed via link: https://lindsay-detlie.clientsecure.me or via the Simple Practice Client Portal App on your phone and the primary email you provide in your paperwork.
Cancellation Policy
Twenty-four hours notice should be given if the client needs to cancel their therapy appointment. The client will be responsible for the full cost of their session or a $100 fee (whichever is lower) in the event of a missed appointment without appropriate notice.
This fee may be waived in the event of a sudden illness or other emergency outside of your control.
This therapist reserves the right to discontinue services after two “no-shows” in a row.
Payment Policy
Payment for services is due within 24 hours of service delivery and is accepted via check, cash, credit card via SimplePractice, or through Venmo for Business to @lindsay-detlie-lcsw.
Invoices are available upon request.
A third party biller, Headway, is utilized for payment services to accommodate certain insurances.
All fees are subject to change and can be reviewed on the attached Services and Fees list.
Emergencies and After Hours Communication
This therapist is unable to provide 24/7 availability for crisis situations. Business hours are considered 8:30am - 5:30pm Monday - Thursday. Any emails, texts, or calls received outside of these hours will be addressed within the next two business days. If you are in crisis, particularly unable to keep yourself safe, then you should go to the nearest emergency room, call 911, or call Region Ten’s 24/7 crisis services line at 434-972-1800.
Telehealth Services Informed Consent
Telehealth psychotherapy services are available through SimplePractice client portal.
If you chose to use telehealth in lieu of in-person sessions or need to utilize telehealth due to various barriers to in-person meetings, then there are several things to consider:

There are benefits and risks to telehealth services including: disruptions to services due to technology failures, interruptions or breaches of confidentiality to unauthorized persons, and limited capacity to address crises or emergencies.

Privacy laws that protect a client’s health information also apply to telehealth services. The same limits of confidentiality apply as well.

You may be asked to resume in-person sessions if safety issues arise that require a higher level of care.

You will be asked to confirm your location and that you are in a safe and private space each session as a precaution in the event of a life threatening emergency.

In the event of a life threatening emergency, this therapist will contact the appropriate authorities and/or your emergency contact on file in order to ensure your safety.
